{"id":10646,"date":"2016-11-18T08:18:29","date_gmt":"2016-11-18T08:18:29","guid":{"rendered":"http:\/\/wames.org.uk\/cms-english\/?p=10646"},"modified":"2016-11-18T08:18:29","modified_gmt":"2016-11-18T08:18:29","slug":"cfs-and-chronic-widespread-pain-in-adolescence","status":"publish","type":"post","link":"https:\/\/wames.org.uk\/cms-english\/cfs-and-chronic-widespread-pain-in-adolescence\/","title":{"rendered":"CFS and chronic widespread pain in adolescence"},"content":{"rendered":"<h3><strong>Research highlights:<\/strong><\/h3>\n<ul>\n<li><span style=\"color: #000000;\">14.6% of adolescents with CFS also experienced chronic widespread pain<\/span><\/li>\n<li><span style=\"color: #000000;\">Females twice as likely to have CFS or CWP<\/span><\/li>\n<li><span style=\"color: #000000;\">Exclusive CFS (vs non-CFS) was associated with higher levels of reported pain and greater effect of pain<\/span><\/li>\n<li><span style=\"color: #000000;\">This association attenuated after adjustment for <a href=\"http:\/\/medical-dictionary.thefreedictionary.com\/covariate\" target=\"_blank\">covariates<\/a> (e.g. depression, anxiety, obesity)<\/span><\/li>\n<li><span style=\"color: #000000;\">Higher levels of depressive &amp; anxiety symptoms are associated with higher odds of <a href=\"http:\/\/medical-dictionary.thefreedictionary.com\/comorbid\" target=\"_blank\">comorbid<\/a> CFS &amp; CWP<\/span><\/li>\n<\/ul>\n<h3><strong>Research abstract:<\/strong><\/h3>\n<p>Whilst many studies have investigated the overlap between pain\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Genotype-phenotype_distinction\" target=\"_blank\">phenotypes<\/a> and chronic fatigue syndrome (CFS) in adults, little is known\u00a0about the relationship between these conditions in adolescents.<\/p>\n<h3>The\u00a0study&#8217;s aim was therefore to identify whether a relationship exists\u00a0between chronic widespread pain (CWP) and CFS in adolescents and\u00a0investigate whether the two share common associations with a set of\u00a0covariates.<\/h3>\n<h3>A questionnaire was administered to offspring of the <a href=\"http:\/\/www.bristol.ac.uk\/alspac\/\" target=\"_blank\">Avon\u00a0Longitudinal Study of Parents and Children<\/a> at age 17, asking about site,\u00a0duration, and pain intensity, from which participants with CWP were\u00a0identified. At the same research clinic, a <a href=\"http:\/\/www.sciencedirect.com\/science\/article\/pii\/S0890856709652771\" target=\"_blank\">computer-based Revised\u00a0Clinical Interview Schedule <\/a>(CIS-R) was filled out, from which a<br \/>\nclassification of CFS was obtained.<\/h3>\n<h3>The relationship between selected\u00a0covariates and CFS and CWP was investigated using a variety of logistic,\u00a0ordinal logistic and multinomial regressions. We identified 3214\u00a0adolescents with complete data for all outcomes and covariates. There\u00a0were 82 (2.6%) individuals classified as CFS and 145 (4.5%) as CWP. A\u00a0classification of CFS resulted in an increased likelihood of having CWP\u00a0(OR: 3.87; 95% CI: 2.05-7.31). Females were approximately twice as\u00a0likely to have CFS or CWP, with multinomial regression revealing a\u00a0greater sex-effect for CWP compared to CFS. Those with exclusive CFS\u00a0were more likely to report higher levels of pain and greater effect of<br \/>\npain compared to those without CFS, though associations attenuated to\u00a0the null after adjustment for covariates, which did not occur in those\u00a0with exclusive CWP.<\/h3>\n<h3>Multinomial regression revealed that relative to\u00a0having neither CFS nor CWP, a one-unit increase in the depression and\u00a0anxiety scales increased the risk of having exclusive CFS and, to a\u00a0greater extent, the risk of having co-morbid CFS and CWP, but not\u00a0exclusive CWP, which was only related to anxiety.<\/h3>\n<p><strong>Perspective:<\/strong><br \/>\nIn this cohort, 14.6% of adolescents with CFS have co-morbid CWP. The likely greater proportion of more mild cases observed in this epidemiological study means that prevalence of overlap may be underestimated compared to those attending specialist services.
