A scoping review<\/a>, by\u00a0Victoria Strassheim, Rebecca Lambson, Katie L. Hackett,\u00a0Julia L. Newton <span style=\"text-decoration: underline;\">in<\/span> <em>Fatigue: Biomedicine, Health &amp; Behavior<\/em> [Preprint\u00a0June 19, 2017]<\/p>\n<p><strong>Background:<\/strong><br \/>\nChronic fatigue syndrome\/myalgic encephalomyelitis (CFS\/ME) affects 0.4%\u00a0of the population. It is characterised by disabling fatigue and a\u00a0combination of self-reported symptoms which include impairments in\u00a0concentration, short-term memory, sleep disturbances, post-exertional\u00a0malaise and musculoskeletal pain.<\/p>\n<p>There are four categories of severity in the CFS\/ME population: mild; moderate; severe; and very severe which\u00a0are expanded on elsewhere and adopted by the National Institute for\u00a0Health and Care Excellence [NICE. Chronic fatigue syndrome\/myalgic<br \/>\nencephelomyelitis diagnosis and management in adults and children;\u00a02007].<\/p>\n<p><strong>Objective:<\/strong><br \/>\nIdentify research relating to those severely and very severely affected\u00a0by CFS\/ME.<\/p>\n<p><strong>Methods:<\/strong><br \/>\nWe searched electronic databases for relevant studies using pre-defined\u00a0search terms: &#8216;chronic fatigue syndrome&#8217; and &#8216;severe&#8217; which covers\u00a0&#8216;severe&#8217; and &#8216;severely&#8217;. Included were English language papers published\u00a0in full that discretely identified severely and very severely affected\u00a0CFS\/ME populations from the broader CFS\/ME population.<\/p>\n<p><strong>Results:<\/strong><br \/>\nOver 2000 papers were reviewed and 21 papers met the selection criteria.\u00a0 The capture produced both adult and paediatric populations with a\u00a0variety of methodologies. Wide differences in illness characterisation,\u00a0definition and measurement were found. Case studies reported that in\u00a0extreme presentations very severe CFS\/ME individuals may be confined to\u00a0bed, requiring reduced light and noise exposure.<\/p>\n<p><strong>Conclusion:<\/strong><br \/>\nThis review highlights the limited research focusing on the severely\u00a0affected CFS\/ME population. The heterogeneity of the condition\u00a0contributes to the lack of consensus concerning definitive diagnostic\u00a0criteria and functional measures to assess disability.
