To gain insight into the pathophysiology of ME\/CFS in New Zealand, we examined the <a href=\"https:\/\/en.wikipedia.org\/wiki\/Transcriptome\" target=\"_blank\" rel=\"noopener\">transcriptomes<\/a> of peripheral blood mononuclear cells by <a href=\"https:\/\/en.wikipedia.org\/wiki\/RNA\" target=\"_blank\" rel=\"noopener\">RNA<\/a>-seq analysis in a small well-characterized patient group (10 patients), with age\/gender-matched healthy controls (10 control subjects).<\/p>\n<p><a href=\"http:\/\/wames.org.uk\/cms-english\/2019\/02\/changes-in-the-transcriptome-of-circulating-immune-cells-of-a-n-z-cohort-with-me-cfs\/gene-transcripts-tate-2019\/\" rel=\"attachment wp-att-19490\"><img data-recalc-dims=\"1\" decoding=\"async\" class=\"aligncenter wp-image-19490 size-full lazyload\" data-src=\"https:\/\/i0.wp.com\/wames.org.uk\/cms-english\/wp-content\/uploads\/2019\/02\/Gene-transcripts-Tate-2019.jpg?resize=620%2C535\" alt=\"\" width=\"620\" height=\"535\" data-srcset=\"https:\/\/i0.wp.com\/wames.org.uk\/cms-english\/wp-content\/uploads\/2019\/02\/Gene-transcripts-Tate-2019.jpg?w=620&amp;ssl=1 620w, https:\/\/i0.wp.com\/wames.org.uk\/cms-english\/wp-content\/uploads\/2019\/02\/Gene-transcripts-Tate-2019.jpg?resize=150%2C129&amp;ssl=1 150w, https:\/\/i0.wp.com\/wames.org.uk\/cms-english\/wp-content\/uploads\/2019\/02\/Gene-transcripts-Tate-2019.jpg?resize=300%2C259&amp;ssl=1 300w\" data-sizes=\"(max-width: 620px) 100vw, 620px\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" style=\"--smush-placeholder-width: 620px; --smush-placeholder-aspect-ratio: 620\/535;\" \/><\/a><\/p>\n<p>Twenty-seven <a href=\"https:\/\/en.wikipedia.org\/wiki\/Transcription_(biology)\" target=\"_blank\" rel=\"noopener\">gene transcripts<\/a> were increased 1.5- to sixfold and six decreased three- to sixfold in the patient group (P\u2009&lt;\u20090.01). The top enhanced gene transcripts, IL8, NF\u039aBIA and TNFAIP3, are functionally related to inflammation, and significant changes were validated for IL8 and NF\u039aBIA by quantitative <a href=\"https:\/\/en.wikipedia.org\/wiki\/Polymerase_chain_reaction\" target=\"_blank\" rel=\"noopener\">polymerase chain reaction<\/a> (qPCR).<\/p>\n<p>Functional network analysis of the altered gene transcripts (P\u2009&lt;\u20090.01) detected interactions between the products related to inflammation, <a href=\"https:\/\/en.wikipedia.org\/wiki\/Circadian_clock\" target=\"_blank\" rel=\"noopener\">circadian clock function<\/a>, <a href=\"http:\/\/complicatedpain.com\/complicated-pain-recovery\/metabolic-dysregulation\/\" target=\"_blank\" rel=\"noopener\">metabolic dysregulation<\/a>, <a href=\"https:\/\/en.wikipedia.org\/wiki\/Cellular_stress_response\" target=\"_blank\" rel=\"noopener\">cellular stress responses<\/a> and <a href=\"https:\/\/alevelbiology.co.uk\/notes\/mitochondria-structure-and-functions\/\" target=\"_blank\" rel=\"noopener\">mitochondrial function<\/a>. Ingenuity pathway analysis (P\u2009&lt;\u20090.05) provided further insights into the dysfunctional physiology, highlighting stress and inflammation pathways.<\/p>\n<p>This analysis provides novel insights into the molecular changes in ME\/CFS and contributes to the understanding of the pathophysiological mechanisms of the disease.<\/p>\n<p><strong>From the conclusion:<\/strong><\/p>\n<p>Our exploratory approach has enabled us to obtain a rich differentially expressed gene dataset to identify changed biology in ME\/CFS. We have identified the circadian rhythm dysregulation pathway as a new possible underlying cause of the unrefreshing sleep, fatigue and metabolic abnormalities seen in ME\/CFS. Furthermore, impaired mitochondrial function and resulting oxidative stress, coupled with chronic immune-inflammatory signalling, provides a compelling explanation for the fatigue, cognitive dysfunction and post-exertion malaise experienced in ME\/CFS.<\/p>\n<p>Therefore, this study is a further step towards gaining an understanding of the disease process and identifying putative biomarkers to support clinical diagnosis.
