Supplement 1; Abingdon Vol. 7,\u00a0\u00a0(2018): 139.<\/h3>\n<p>&nbsp;<\/p>\n<h3><strong>Research abstract:<\/strong><\/h3>\n<p><strong>Background: <\/strong><\/p>\n<p>ME\/CFS (ICD-10; G93.3) is a complex multisystem disease\u00a0of unknown origin with characteristic clinical features that include postexertional\u00a0malaise, cognitive dysfunction, orthostatic intolerance, ongoing\u00a0flu-like symptoms and unrefreshing sleep in conjunction with\u00a0other. Its worldwide prevalence is 0.4%\u20131% with a female to male ratio\u00a0of 6:1. Current treatments rely on the management of symptoms due to\u00a0a lack of understanding of the underlying mechanisms of disease onset\u00a0and progression. The aim of this work was to identify biomarkers of\u00a0ME\/CFS by analysing miRNA profiles of patient plasma EVs and comparing\u00a0them to those of their PBMCs. This information should improve\u00a0our knowledge of ME\/CFS and allow the development of unbiased\u00a0quantitative diagnostic methods.<\/p>\n<p><strong>Methods: <\/strong><\/p>\n<p>miRNA profiles of PBMCs or EVs isolated from plasma\u00a0(Invitrogen cat.4484450) of\u00a0 ME\/CFS patients and population, sex, age\u00a0and BMI-matched healthy participants (N = 15 per group) from the\u00a0ME UK Biobank (London, UK) were determined using Nanostring\u00a0technology (nCounter Human v3 miRNA Expression Assay Kit).<\/p>\n<p>Gene ontology (GO) and the Kyoto encyclopedia of genes and genomes\u00a0(KEGG) were used to determine disrupted cellular functions in\u00a0ME\/CFS. The study was approved by the DGSP-CSISP CEIC (ref.\u00a0UCV201701), Spain. Signed informed consent was required for inclusion\u00a0of samples.<\/p>\n<p><strong>Results:<\/strong><\/p>\n<p>miRNA profiles evidenced a global trend for miRNA downregulation\u00a0in patients with respect to healthy controls (76% and 64% of\u00a0the miRNAs presented inhibition, by at least 50%, in PBMCs and EVs\u00a0respectively; while only one miRNA in PBMCs and 6% of them in EVs\u00a0showed upregulation to this level). Qualitatively, miRNA profiles in\u00a0PBMCs did not match those obtained from EVs indicating active packaging\u00a0of miRNAs in EVs. The functions to be affected by the deregulated\u00a0miRNAs support a model of immune, mitochondrial and neural defects\u00a0for this disorder.<\/p>\n<p><strong>Summary\/Conclusion:<\/strong><\/p>\n<p>This is the first report of paired PBMCs and EV\u00a0miRNA profiles of ME\/CFS patients by enzyme-free array technology.\u00a0The results confirm previous proposals that this epigenetic mechanism is\u00a0linked to the pathophysiology of ME\/CFS.
