Along with disabling fatigue, ME\/CFS presents an array of other core symptoms, including autonomic nervous system (ANS) dysfunction, sustained inflammation, altered energy metabolism, and mitochondrial dysfunction.<\/p>\n<p><img data-recalc-dims=\"1\" decoding=\"async\" class=\"alignright wp-image-31773 lazyload\" data-src=\"https:\/\/i0.wp.com\/wames.org.uk\/cms-english\/wp-content\/uploads\/2021\/07\/biomolecules-11-00961.jpg?resize=350%2C280&#038;ssl=1\" alt=\"\" width=\"350\" height=\"280\" data-srcset=\"https:\/\/i0.wp.com\/wames.org.uk\/cms-english\/wp-content\/uploads\/2021\/07\/biomolecules-11-00961.jpg?resize=300%2C240&amp;ssl=1 300w, https:\/\/i0.wp.com\/wames.org.uk\/cms-english\/wp-content\/uploads\/2021\/07\/biomolecules-11-00961.jpg?resize=150%2C120&amp;ssl=1 150w, https:\/\/i0.wp.com\/wames.org.uk\/cms-english\/wp-content\/uploads\/2021\/07\/biomolecules-11-00961.jpg?w=641&amp;ssl=1 641w\" data-sizes=\"(max-width: 350px) 100vw, 350px\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" style=\"--smush-placeholder-width: 350px; --smush-placeholder-aspect-ratio: 350\/280;\" \/>Here, we evaluated patients&#8217; symptomatology and the mitochondrial metabolic parameters in <a href=\"https:\/\/en.wikipedia.org\/wiki\/Peripheral_blood_mononuclear_cell\" target=\"_blank\" rel=\"noopener\">peripheral blood mononuclear cells<\/a> (PBMCs) and <a href=\"https:\/\/me-pedia.org\/wiki\/Plasma\" target=\"_blank\" rel=\"noopener\">plasma<\/a> from a clinically well-characterised cohort of six ME\/CFS patients compared to age- and gender-matched controls.<\/p>\n<p>We performed a comprehensive cellular assessment using bioenergetics (<a href=\"https:\/\/www.agilent.com\/en\/support\/cell-analysis\/what-does-xf-tech-measure\" target=\"_blank\" rel=\"noopener\">extracellular flux analysis<\/a>) and protein profiles (<a href=\"https:\/\/en.wikipedia.org\/wiki\/Quantitative_proteomics\" target=\"_blank\" rel=\"noopener\">quantitative mass spectrometry-based proteomics<\/a>) together with self-reported symptom measures of fatigue, ANS dysfunction, and overall physical and mental well-being.<\/p>\n<p>This ME\/CFS cohort presented with severe fatigue, which correlated with the severity of ANS dysfunction and overall physical well-being. PBMCs from ME\/CFS patients showed significantly <strong>lower mitochondrial coupling efficiency<\/strong>. They exhibited <a href=\"https:\/\/en.wikipedia.org\/wiki\/Proteome\" target=\"_blank\" rel=\"noopener\">proteome<\/a> alterations, including altered <a href=\"https:\/\/me-pedia.org\/wiki\/Mitochondrion\" target=\"_blank\" rel=\"noopener\">mitochondrial<\/a> metabolism, centred on pyruvate dehydrogenase and coenzyme A metabolism, leading to a decreased capacity to provide adequate intracellular ATP levels.<\/p>\n<p>Overall, these results indicate that PBMCs from ME\/CFS patients have a decreased ability to fulfill their cellular energy demands.<\/p>\n<h3><strong>From study Discussion (4):<\/strong><\/h3>\n<p>&#8230;in this study, we aimed to recruit a homogenous group of ME\/CFS patients and applied strategies for personalised medicine by performing many tests in a few individuals to shed light on molecular mechanisms of disease in these individuals.<\/p>\n<p>Despite the small sample size, our data indicate a <strong>dysregulated mitochondrial metabolism<\/strong> centred on PDH and CoA metabolism, which supports findings from other and larger ME\/CFS cohorts [29].<\/p>\n<p>Interestingly, between 35 and 40% of ME\/CFS patients experience significant improvement in their health when treated with <a href=\"https:\/\/me-pedia.org\/wiki\/Dichloroacetic_acid\" target=\"_blank\" rel=\"noopener\">dichloroacetate<\/a>, a well-known activator of PDH [74,75]. These open-label studies need further follow-up; however, together with the present study and the previous metabolomics and proteomic studies of independent ME\/CFS cohorts, they support dysregulated mitochondrial metabolism as an important feature of ME\/CFS pathology.<\/p>\n<p>To some degree, abnormalities in bioenergetics parameters of blood cells have shown to correlate with the severity of ME\/CFS symptoms [31,76].
