{"id":32771,"date":"2021-11-03T21:18:29","date_gmt":"2021-11-03T21:18:29","guid":{"rendered":"https:\/\/wames.org.uk\/cms-english\/?p=32771"},"modified":"2021-11-03T21:24:34","modified_gmt":"2021-11-03T21:24:34","slug":"research-review-cfs-cardiovascular-disease","status":"publish","type":"post","link":"https:\/\/wames.org.uk\/cms-english\/research-review-cfs-cardiovascular-disease\/","title":{"rendered":"Research review: CFS &#038; cardiovascular disease"},"content":{"rendered":"<h3><a href=\"https:\/\/www.jacc.org\/doi\/abs\/10.1016\/j.jacc.2021.06.045\" target=\"_blank\" rel=\"noopener\">Chronic Fatigue Syndrome and cardiovascular disease: JACC state-of-the-art review<\/a>, by\u00a0Benjamin H Nateleson, Danielle L Brunjes, Donna Mancini <span style=\"text-decoration: underline;\">in<\/span> <em>Journal of the American College of Cardiology <\/em>Vol 78, #10, pp 1056\u20131067, Sep 7, 2021<\/h3>\n<p>&nbsp;<\/p>\n<h3><strong>Review highlights<\/strong><\/h3>\n<ul>\n<li>ME\/CFS, a clinical entity of unknown etiology characterized by PEM, is easily diagnosed when following available guidelines.<\/li>\n<li>Patients with ME\/CFS typically have small hearts, low stroke volume, and low total blood volume, and some have OI.<\/li>\n<li>More studies are needed to understand the role of deconditioning in producing some of the characteristics of ME\/CFS.<\/li>\n<\/ul>\n<p><img data-recalc-dims=\"1\" decoding=\"async\" class=\"aligncenter size-full wp-image-32773 lazyload\" data-src=\"https:\/\/i0.wp.com\/wames.org.uk\/cms-english\/wp-content\/uploads\/2021\/09\/Natelson-2021-scaled-e1630997705115.jpg?resize=800%2C511&#038;ssl=1\" alt=\"\" width=\"800\" height=\"511\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" style=\"--smush-placeholder-width: 800px; --smush-placeholder-aspect-ratio: 800\/511;\" \/><\/p>\n<p><strong>Review abstract:<\/strong><\/p>\n<p>Myalgic encephalomyelitis\/chronic fatigue syndrome (ME\/CFS) is a medically unexplained illness characterized by severe fatigue limiting normal daily activities for at least 6 months accompanied by problems with unrefreshing sleep, exacerbation of symptoms following physical or mental efforts (<a href=\"https:\/\/me-pedia.org\/wiki\/Post-exertional_malaise\" target=\"_blank\" rel=\"noopener\">postexertional malaise<\/a> [PEM]), and either <a href=\"https:\/\/me-pedia.org\/wiki\/Cognitive_dysfunction\" target=\"_blank\" rel=\"noopener\">cognitive reports<\/a> or physiological evidence of <a href=\"https:\/\/me-pedia.org\/wiki\/Orthostatic_intolerance\" target=\"_blank\" rel=\"noopener\">orthostatic intoleranc<\/a>e in the form of either orthostatic tachycardia and\/or hypocapnia.<\/p>\n<p>Although rarely considered to have <a href=\"https:\/\/me-pedia.org\/wiki\/Cardiac_problems_in_ME\/CFS\" target=\"_blank\" rel=\"noopener\">cardiac dysfunction<\/a>, ME\/CFS patients frequently have <a href=\"https:\/\/en.wikipedia.org\/wiki\/Stroke_volume\" target=\"_blank\" rel=\"noopener\">reduced stroke volume<\/a> with a significant inverse relation between cardiac output and PEM severity. Magnetic resonance imaging of ME\/CFS patients compared with normal control subjects found significantly reduced stroke, <a href=\"https:\/\/en.wikipedia.org\/wiki\/End-systolic_volume#:~:text=End%2Dsystolic%20volume%20(ESV),point%20in%20the%20cardiac%20cycle.\" target=\"_blank\" rel=\"noopener\">end-systolic<\/a>, and end-diastolic volumes together with reduced end-diastolic wall mass. Another cardiovascular abnormality is reduced nocturnal blood pressure assessed by 24-hour monitoring.<\/p>\n<p>Autonomic dysfunction is also frequently observed with postural orthostatic tachycardia and\/or hypocapnia.
