We used a 10-min passive vertical <a href=\"https:\/\/batemanhornecenter.org\/wp-content\/uploads\/filebase\/providers\/mecfs\/10-Minute-NASA-Lean-Test-Clinician-Instructions-06_2021.pdf\" target=\"_blank\" rel=\"noopener\">lean test<\/a> as <a href=\"https:\/\/link.springer.com\/chapter\/10.1007\/978-1-4471-4978-1_12\" target=\"_blank\" rel=\"noopener\">orthostatic challenge<\/a>-OC and measured changes in vitals and end tidal CO2\u00a0(eTCO2). An abnormal physiologic response to OC was identified in 60% of the 63 patients evaluated from one to three times over several years. <a href=\"https:\/\/en.wikipedia.org\/wiki\/Hypocapnia\">Hypocapnia<\/a>, either resting or induced by OC, was the most frequent abnormality, followed by <a href=\"https:\/\/me-pedia.org\/wiki\/Postural_orthostatic_tachycardia_syndrome\" target=\"_blank\" rel=\"noopener\">postural orthostatic tachycardia.<\/a><\/p>\n<p><strong>Objective<\/strong><br \/>\nEvaluate the physiologic response of patients with ME\/CFS to a standardized OC.<\/p>\n<p><strong>Design<\/strong><br \/>\nRespiratory and heart rate, blood pressure and eTCO2\u00a0were recorded twice at the end of 10-min supine rest and then every minute during the 10-min lean. Hypocapnia was eTCO2\u2009\u2264\u200932\u00a0mmHg. Orthostatic tachycardia was heart rate increase\u2009\u2265\u200930 beats per minute\u00a0compared with resting or\u2009\u2265\u2009120 BPM. <a href=\"https:\/\/www.betterhealth.vic.gov.au\/health\/conditionsandtreatments\/dizziness-orthostatic-hypotension\" target=\"_blank\" rel=\"noopener\">Orthostatic hypotension<\/a> was decreased systolic pressure\u2009\u2265\u200920\u00a0mmHg from baseline. <a href=\"https:\/\/en.wikipedia.org\/wiki\/Tachypnea\" target=\"_blank\" rel=\"noopener\">Tachypnea<\/a> was respiratory rate of\u00a0\u2009\u2265\u200920 breaths per minute\u2014either supine or leaning. Questionnaire data on symptom severity, quality of life and mood were collected at visit #2.<\/p>\n<p><strong>Patients<\/strong><br \/>\n63 consecutive patients fulfilling the 1994 case definition for CFS underwent lean testing at first visit and then annually at visit 2 (n\u2009=\u200948) and 3 (n\u2009=\u200929).<\/p>\n<p><strong>Measures<\/strong><br \/>\nSupine hypocapnia; orthostatic tachycardia, hypocapnia or hypotension.<\/p>\n<p><strong>Results<\/strong><br \/>\nThe majority of ME\/CFS patients (60.3%, 38\/63) had an abnormality detected during a lean test at any visit (51%, 50% and 45% at visits 1, 2 and 3, respectively). <a href=\"https:\/\/en.wikipedia.org\/wiki\/Hypocapnia\" target=\"_blank\" rel=\"noopener\">Hypocapnia<\/a> at rest or induced by OC was more common and more likely to persist than postural orthostatic tachycardia. Anxiety scores did not differ between those with and without hypocapnia.<\/p>\n<p><strong>Conclusions<\/strong><br \/>\nThe 10-min lean test is useful in evaluation of OI in patients with ME\/CFS.
