People with ME\/CFS, their partners and healthcare professionals are unable to make informed, evidence-based decisions around family planning, pregnancy, labour and birth.<\/p>\n<p>&#8220;More research is urgently needed, considering all aspects of pregnancy and ME\/CFS for patients and healthcare professionals. In particular, research should explore what would constitute high-quality care for those with ME\/CFS relating to pregnancy and family planning.&#8221;<\/p>\n<p>Better quality research is also needed, with larger sample sizes, healthy control groups, clearly defined research questions and a standard diagnostic criteria for ME\/CFS.<\/p>\n<p>&nbsp;<\/p>\n<p><a href=\"https:\/\/bmjopen.bmj.com\/content\/13\/10\/e070366\" target=\"_blank\" rel=\"noopener\">Identifying, synthesising and appraising existing evidence relating to myalgic encephalomyelitis\/chronic fatigue syndrome and pregnancy: a mixed-methods systematic review<\/a>, by <span class=\"name\">Emma Slack<\/span>, <span class=\"name\">Katrina Anne Pears<\/span>, <span class=\"name\">Judith Rankin<\/span>, <span class=\"name\">Julia L Newton<\/span>, M<span class=\"name\">ark Pearce <span style=\"text-decoration: underline;\">in<\/span> <em>BMJ Open <\/em>Volume 13, Issue 10, 2023<\/span><\/p>\n<div class=\"panel-pane pane-highwire-panel-tabs-container col-narrow-12 clear\">\n<div class=\"pane-content\">\n<div id=\"panels-ajax-tab-container-highwire_article_tabs\" class=\"panels-ajax-tab-container\" data-panels-ajax-tab-preloaded=\"jnl_template_bmjj_tab_art\">\n<div class=\"panels-ajax-tab-wrap-jnl_template_bmjj_tab_art\">\n<div class=\"panel-display panel-1col clearfix\">\n<div class=\"panel-panel panel-col\">\n<div class=\"panel-pane pane-highwire-markup abstract-with-bc\">\n<div class=\"pane-content\">\n<div class=\"highwire-markup\">\n<div id=\"content-block\">\n<div class=\"article abstract-view \">\n<div id=\"abstract-1\" class=\"section abstract\">\n<div id=\"sec-1\" class=\"subsection\">\n<p><strong>Research abstract: <\/strong><\/p>\n<p><strong>Objectives<\/strong><\/p>\n<p>To identify, synthesise and appraise evidence relating to myalgic encephalomyelitis\/chronic fatigue syndrome (ME\/CFS) and pregnancy.<\/p>\n<\/div>\n<div id=\"sec-2\" class=\"subsection\">\n<p id=\"p-3\"><strong>Design<\/strong><\/p>\n<p>Mixed-methods systematic review, using convergent segregated design.<\/p>\n<\/div>\n<div id=\"sec-3\" class=\"subsection\">\n<p id=\"p-4\"><strong>Data sources<\/strong><\/p>\n<p>MEDLINE, EMBASE, Scopus, PsycINFO, CINAHL, MedRxiv, PROSPERO and grey literature sources through 6 August 2023.<\/p>\n<\/div>\n<div id=\"sec-4\" class=\"subsection\">\n<p id=\"p-5\"><strong>Eligibility criteria<\/strong><\/p>\n<p>We included original research studies, expert opinion and grey literature reporting on ME\/CFS and pregnancy\/post partum (up to 2\u2009years), risk of pregnancy outcomes with ME\/CFS or experiences during pregnancy for mother, partner or health and social care professionals following ME\/CFS during pregnancy, all where the evidence was relevant to a confirmed ME\/CFS diagnosis prior to pregnancy.<\/p>\n<\/div>\n<div id=\"sec-5\" class=\"subsection\">\n<p id=\"p-6\"><strong>Data extraction and synthesis<\/strong><\/p>\n<p>Three independent reviewers completed all screening, data extraction and quality assessment. Risk of bias was assessed using the mixed-methods appraisal tool V.2018. <a href=\"https:\/\/en.wikipedia.org\/wiki\/Qualitative_research\" target=\"_blank\" rel=\"noopener\">Qualitative<\/a> and <a href=\"https:\/\/www.scribbr.co.uk\/research-methods\/introduction-to-quantitative-research\/#:~:text=Quantitative%20research%20is%20the%20process,generalise%20results%20to%20wider%20populations.\" target=\"_blank\" rel=\"noopener\">quantitative<\/a> literature was analysed separately using thematic and descriptive syntheses. Findings were integrated through <a href=\"https:\/\/open.ncl.ac.uk\/theories\/11\/configurational-theory\/#:~:text=Configurational%20Theory%20(CT),with%20the%20outcome%20of%20interest.\" target=\"_blank\" rel=\"noopener\">configuration<\/a>.<\/p>\n<\/div>\n<div id=\"sec-6\" class=\"subsection\">\n<p id=\"p-7\"><strong>Results<\/strong><\/p>\n<p>Searches identified 3675 articles, 16 met the inclusion criteria: 4 quantitative (1 grey), 11 qualitative (9 grey) and 1 grey mixed-methods study. Of the four quantitative studies that reported on ME\/CFS severity during pregnancy:<\/p>\n<ul>\n<li>two suggested pregnancy negatively impacted on ME\/CFS<\/li>\n<li>one found most women had no change in ME\/CFS symptoms and<\/li>\n<li>one found ME\/CFS improved;<\/li>\n<\/ul>\n<p>this difference in symptom severity across studies was supported by the qualitative evidence.<\/p>\n<p>The qualitative literature also highlighted the importance of individualised care throughout pregnancy and birth, and the need for additional support during family planning, pregnancy and with childcare.<\/p>\n<p>Only one quantitative study reported on pregnancy outcomes, finding decreased vaginal births and higher rates of spontaneous abortions and developmental and learning delays associated with pregnancies in those with ME\/CFS.<\/p>\n<\/div>\n<div id=\"sec-7\" class=\"subsection\">\n<p id=\"p-8\"><strong>Conclusions<\/strong><\/p>\n<p>Current evidence on ME\/CFS in pregnancy is limited and findings inconclusive. More high-quality research is urgently needed to support the development of evidence-based guidelines on ME\/CFS and pregnancy.<\/p>\n<p><strong>STRENGTHS AND LIMITATIONS OF THIS STUDY<\/strong><\/p>\n<ul>\n<li>Thorough and systematic search for both peer-reviewed and grey literature relating to myalgic encephalomyelitis\/ chronic fatigue syndrome and pregnancy that was not limited by date of publication.<\/li>\n<li>Inclusion of both qualitative and quantitative evidence from both peer-reviewed and grey literature sources allowed us to explore different aspects of a complex research question.<\/li>\n<li>Limited evidence was available, particularly peer-reviewed literature, which restricted the conclusions we were able to make in this study.<\/li>\n<\/ul>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n","protected":false},"excerpt":{"rendered":"<p>ME\/CFS and pregnancy: a systematic review &nbsp; When reviewing studies of pregnancy in women living with ME\/CFS, researchers from Newcastle University found the impact of pregnancy on ME\/CFS severity varied within and between the small number of relevant studies.
