The Institute of Medicine (IOM) report has\u00a0proposed new clinical criteria and a new name for this illness, but\u00a0aspects of these recommendations have been scrutinized by patients and<br \/>\nscientists.<\/p>\n<p><strong>Purpose:<\/strong><br \/>\nIt is possible that both <a href=\"http:\/\/www.thefreedictionary.com\/empiric\" target=\"_blank\">empiric<\/a> and <a href=\"https:\/\/en.wikipedia.org\/wiki\/Consensus_decision-making\" target=\"_blank\">consensus<\/a> approaches could be used\u00a0to help settle some of these diagnostic challenges. Using patient\u00a0samples collected in the USA, Great Britain, and Norway (N\u2009=\u2009556), the\u00a0current study attempted to categorize patients using more general as\u00a0well as more restricted case definitions.<\/p>\n<p><strong>Results:<\/strong><br \/>\nOverall, the outcomes suggest that there might be four groupings of\u00a0patients, with the broadest category involving those with chronic\u00a0fatigue (N\u2009=\u200962), defined by six or more months of fatigue which cannot\u00a0be explained by medical or psychiatric conditions. A second category\u00a0involves those patients who have chronic fatigue that can be explained<br \/>\nby a medical or psychiatric condition (N\u2009=\u200947).<\/p>\n<p>A third category\u00a0involves more specific criteria that have been posited both by the IOM\u00a0report, Canadian Clinical Case criteria, ME-ICC criteria and a more\u00a0empiric approach. These efforts have specified domains of substantial\u00a0reductions of activity, post-exertional malaise, neurocognitive\u00a0 impairment, and sleep dysfunction (N\u2009=\u2009346). Patients with these\u00a0characteristics were more functionally impaired than those\u00a0meeting\u00a0just\u00a0chronic fatigue criteria, p\u2009&lt;\u2009.05. Finally, those meeting even more\u00a0restrictive ME criteria proposed by Ramsay, identified a smaller and\u00a0even more impaired group, p\u2009&lt;\u2009.05.<\/p>\n<p><strong>Conclusion:<\/strong><br \/>\nIt is important that scientists world-wide develop consensus on how to\u00a0identify and classify patients using clinical and research criteria, and\u00a0ultimately develop subtypes within such categories.<\/p>\n<p><a href=\"http:\/\/www.tandfonline.com\/doi\/full\/10.1080\/21641846.2015.1124520\" target=\"_blank\">Case definitions integrating empiric and consensus perspectives<\/a>, by\u00a0Leonard A.
