The North Wales Community Health Council (CHC) and the Community Transport Association Wales (CTA) are working together to gain feedback concerning how people are able to get to their NHS appointments.

They would like to hear comments on all aspects of the current transport system be it public; community or Welsh Ambulance patient transport.

Your comments will be shared with the North Wales Transport to Health Group and will be used as valuable feedback from people who use the transport services.
The North Wales Transport to Health Group is one of three working groups across Wales set up by Welsh Government to look at how patients, their families and carers, view their journeys to hospital.

All feedback will help to improve health services in North Wales.
